If you use the state's SCAN long distance network for phone calls then include your "DIS Agency Billing ID". If you don't use SCAN a new "DIS Agency Billing ID" number will be assigned. The Off-K-20 Network Long Distance form can also be faxed to the K20 at 1-888-934-5558.
